Please be aware, all the forms are transitioning to electronically. You may be asked to fill out an iPad when arriving so please arrive early to expedite your visit.
- New Patient Information Form (required for all new patients)
- Demographic Form (required for all new patients to Michigan Orthopedic Institute)
- Pain Form (for each patient visit)
- Neck Pain Disability Index Questionnaire (required at each visit if you had neck surgery with dr park)
- Oswestry Disability Questionnaire (required at each visit if you had back surgery with dr park)
- SF-12 Health Survey
- Swallowing questionnaire (required if you had anterior neck surgery with dr park)
